Online Patient Access

ONLINE ACCESS - MADE EASY! - Update 22.4.20

We can now provide online access for you to order your repeat prescriptions and book appointments very quickly by answering 3 simple identification check questions relating to your medical record at the surgery. There is now no need to provide other forms of identification for these services.
Simply Call us on 016977 2551, answer 3 questions relating to your contact with the surgery which confirms your identification for us.
Within 48 hours an email will be sent to you with a linkage key with a list of all providers for online services to choose from.
When registering on your chosen website you enter the linkage key to activate your account and then your ready to go.
Ordering repeat prescriptions online is simple and provides a quicker and more secure service to you and helps you to keep track of your medication requests. It will also ensure that the telephone order line is kept free for vulnerable patients who do not have the ability to use the online service.

The Practice currently offers access for

Online access to Medical Records - currently not available with verbal verification method.

From 1 April 2016 the practice is able to offer online access to medical records.  This will consist of access to coded information in personal records. 

This information is highly confidential and it is important you understand the safety and security implications of this access as well as your personal responsibility to keep your information secure.  If you are interested in having online access to your medical records please read the briefing information below before progressing further.

To be able to give patients access to their records the practice must confirm the identity of the person requesting access.  This is in addition to any identity checks that may have been carried out in the past and must be provided in person.  The information below outlines the identification checks that will be needed.

Proof of ID and Access Information

If you would like to apply for access to your medical records you may ask for an information pack at reception at any of our branches or alternatively complete the appropriate form(s) below.  These MUST be returned in person, with appropriate ID to BRAMPTON surgery ONLY.  Applications via post or email cannot be accepted and each person requesting access, including for children, must attend in person.

Please note that as there are a number of checks that must be made before online access is granted this may take up to 21 days from sumbission of your application.  For newly registered patients this may take up to 3 months due to the time taken for your full medical records to reach the practice.

Proxy Access Patient Information

Should you wish to proceed with an application for proxy access you may ask for an information pack at any of our branches or alternatively complete the application form below.  As for other types of online access this form MUST be returned in person with appropriate ID for the applicant AND the patient to BRAMPTON surgery ONLY.  Applications via post or email cannot be accepted.  Due to the possibly complex nature of proxy access requests these may take up to 21 days from submission of the application and may require a GP appointment.
